• Old Passport (Original & Self-attested Photocopy)
  • First two and last two pages
  • ECR/Non-ECR page
  • Page of observation (if any)
  • Valid visa page (if applicable)
  • Spouse's passport copy showing the same address (if residing together post-divorce)
• Proof of Present Address
  Any one of the following:
  

  • Aadhaar Card
  • Electricity Bill/Water Bill/Telephone Bill (last 3 months)
  • Bank Statement (last 12 months, with bank seal and signature)
  • Rent Agreement (registered)
  • Voter ID
  • Gas connection bill/card
• Proof of Date of Birth
  Only if not clearly mentioned in old passport or if change is requested
  

  • Birth Certificate
  • 10th/12th Class Certificate
• Self-Attested Copies of all documents
• Gazette Notification (if major name change)
• Newspaper Advertisements (optional but helpful)
• Change in Date of Birth
  • Birth Certificate from municipal authority
• Affidavit and supporting documents as per passport rules
• Divorce-specific Documents (submit based on change request)
  If spouse name is to be deleted from passport:
  

  • Divorce Decree/Order issued by the court (self-attested copy)
  • Annexure "E" – Affidavit for deletion of spouse's name

  
  If applicant had changed surname after marriage and now wants to revert to maiden name:
  

  • Divorce Decree
  • Affidavit for name change (optional but recommended)
  • Gazette Notification (if major name change)
