  • Birth Certificate of the Minor
    Issued by Municipal Authority/Registrar of Births & Deaths
  • Proof of Present Address
    Any one of the following, in the name of parent:
    • Aadhaar Card
    • Electricity Bill/Water Bill/Telephone Bill (last 3 months)
    • Bank Statement (last 12 months)
    • Rent Agreement (registered)
    • Voter ID of parent
    • Passport of either parent showing the same address
  • Proof of Parents’ Citizenship/Passports
    • Original passports of both or either parent, with self-attested copies.
  • Passport-Sized Photographs
    • Recent photos of the minor (and sometimes parents) per PSK requirements.
  • Annexures (Declarations and Consent)
    • Annexure D – consent form signed by both parents/guardians.
    • Annexure C – required for minors aged 15–18, or in certain special circumstances (e.g., single parent, separated parents).
