Gathering required documents for New Indian Passport for children under 15
- Place order for Child PassportExpediting appointments are very limited. We are processing applications based on when we receive the documents and the dates of travel. To ensure your passport is ready on time, please place the order and forward your documents to us as soon as possible.
- Birth Certificate of the MinorIssued by Municipal Authority/Registrar of Births & Deaths
- Proof of Present AddressAny one of the following, in the name of parent:
- Aadhaar Card
- Electricity Bill/Water Bill/Telephone Bill (last 3 months)
- Bank Statement (last 12 months)
- Rent Agreement (registered)
- Voter ID of parent
- Passport of either parent showing the same address
- Proof of Parents' Citizenship/Passports
- Original passports of both or either parent, with self-attested copies.
- Passport-Sized Photographs
- Recent photos of the minor (and sometimes parents) per PSK requirements.
- Annexures (Declarations and Consent)
- Annexure D – consent form signed by both parents/guardians.
- Annexure C – required for minors aged 15–18, or in certain special circumstances (e.g., single parent, separated parents).
- Additional documentsAdditional documents for re-issue:
- Old Passport (Original & Self-attested Photocopy)
- First two and last two pages
- Page with observation (if any)
- Valid visa page (if applicable)
If single parent is applying:- Supporting legal documents (Divorce decree / Court order / Death certificate etc.)
If guardian (not biological parent) is applying:- Legal Guardianship certificate
- Annexure D signed by guardian
For minors with one or both parents residing abroad:- Parent's passport and visa copies
- Notarized NOC or Annexure D sent from abroad (if not physically present)
