  • Bank or post office statement/passbook copy
  • Rent agreement
  • Employer certificate (for government employees)
• Proof of residence
  Any one of the following:
  

  • Birth Certificate
  • Class X certificate/mark sheet with DOB
  • PAN card
  • Aadhaar Card
  • Driving license with DOB
• Additional documents for Divorcee
  Additional Forms & Annexures:
  

  • Form II – Miscellaneous Services (if making changes post-divorce).
  • Annexure E / Affidavit (Deed Poll) for name change after divorce as required.
  • Passport Application & Photographs
  • Online passport application printout (filled, signed)
  • 2-3 passport-size photographs as per PSK standards
